  Parent Care Management Services

Care ManagementAdult children want their parents to have the best possible care and attention. Providing care for an elderly parent can be deeply rewarding. It can also be emotionally stressful, physically exhausting, and financially draining. The demands on the caregiver can take a toll on their health, career, and relationships.

A large amount of information is available but caregivers often do not have the time to become experts in elder care and difficult decisions often have to be made quickly. As a result, a new profession known as Geriatric Care Manager evolved over the past 15 years to assist seniors and their families in planning, arranging, and monitoring appropriate care for frail, elderly, and disabled people.

What is a Geriatric Care Manager?
A geriatric care manager is a professional who specializes in assisting older people and their families with long-term care arrangements. Working with family members or directly with the elderly client, a geriatric care manager conducts a comprehensive assessment to determine the options for care that best fit the individual.

Parent Care Management Services offers geriatric care management services. One of the many facets of our services is the finding, referring, training, scheduling, and supervising of caregivers to care for the elderly in their homes. PCMS pre-screens caregivers to ensure compatibility, level of background training needed, and the ability to follow the care plans designed for each individual client. Care plans are formulated in conjunction with the family to meet the needs of each client. Each home is provided with an individualized caregiver handbook with special instructions for the care of that specific client, including protocols on contacting the Care Manager regarding concerns about the client's well being.

PCMS clients enjoy the benefit of knowing that we attempt to place caregivers that are familiar with and trained specifically for the care level of that client. (IE, Some caregivers are good companions; others are skilled in more technical aspects of care, like catheter, PEG tube, diabetic monitoring etc.)
